PDF designer

I have a pdf of an invoice pad that I basically want to turn into a form that you can fill in instead of writing out by hand. Is there any free software out there that can do this? basically put fields into an existing form.

 

I've got Acrobat pro but you can't seem to do it in that, do I need Adobe designer?

Cheers skol but it's not Mac compatible. I'll find something, just thought all the IT wizards on here might have had something to hand.

Adobe InDesign is the only thing I can suggest. Okay, it's tantamount to using a sledgehammer to crack a nut but it'll do what you want.

Can you set fields in that? Used it a couple of times but quickly went back to Quark as thats what most the printers we use prefer.
