Occasionally tubes but not with cannulas, as firstly I can get access in almost anyone (I know cannula bragging is what house officers do but I'm quite proud that I put in a 14G with my left hand at half five this morning - although as you would expect the vein would have comfortably accommodated a swann ganz introducer) and secondly because I don't worry about not getting a drip in the same way I do about a tube because obviously the consequences are so much more dire. Although not being able to cannulate a fatty exsanguinating from a lacerated femoral artery wouldn't be ideal, of course. However seeing as most people (including a lot of surgeons, such as the one who tried unsuccessfully to get me to do that man I described above at 4am) are oblivious to a lot of the practical problems associated with obesity, let alone the threat of death from a failed intubation, I think there should be a public health campaign based on what doctors see when they look at the obese. For example, you could easily concoct a story in obstetrics of combined anaesthetic and surgical difficulty leading to tragedy for both mother and baby.

I've already had a shower earlier today, I mean I could do with relaxing in a steaming hot bath (with bubbles and the Les Mis soundtrack on for Dr Gloom) than I really need a clean.

I'll let Monkeys Fist field that one. I could do with a bath, just finished 7 nights and spent most of the day sorting a broken starting motor. At least I've got a week off before my new job starts.

Tbf curries are one of the few meals I usually have (naan) bread with, on the infrequent occasions I have them.

Keep at it CT. All doctors dread obese patients, but none more than the anaesthetist. An otherwise healthy man very nearly died within a few minutes of his anaesthetic starting for a fairly routine operation at my hospital recently, as a direct consequence of him having a BMI of 42. I'm not saying you're obese, I don't know how much you weigh but trust me, you really don't want to be one of those patients that set my spider senses tingling like they do when I see patients like him.

What does that even mean? Dishing 'gay's out left right and centre at the thought of going to see a musical while simultaneously defending your right to harp on about the gym, you literally could not be acting more like a closet puddle jumper. -
Looks even worse that Pardew is making apologies rather than Colo saying something himself.

I think if it was something people would have sympathy for then he'd have made a statement to the fans (he wouldn't even have to say what the issue was if it was something like you suggest) IMO. The absence of such a statement (and the letter to SL fans) makes me extremely reluctant to give him the benefit of the doubt. As SLP says he's taking the piss until there's strong evidence to the contrary, and if it were a viable option I wouldn't want him to play for the club again.

Nowt to do last night so watched Wedding Crashers. A lot funnier than I remembered it. Rachel McAdams is absolutely stunning, reminds me very much of an ex-girlfriend (d'oh!) of mine. Bah. -

Haha, no I don't. We had a young lad the other night for an OGD for bleeding DUs, he properly shat himself afterwards. The female ODPs hid in the anaesthetic room and made the male nurses clear it up "it's a boy problem!".

Amputations aren't that bad. Debridement of necrotising fasciitis (removal of dead, rotting, infected skin, fat and muscle), where the smell hits you as you enter the theatre complex, 20 feet from the patient, is minging. As is the smell of malaena (bleeding from the stomach that has trickled down through the bowels, half digested and mixed in with the shit).

On nights, only had one case (debridement of foot) which we finished hours ago, should really get some kip but need to catch up on my logbook before I leave the trust next week #whythefuckhaveileftittilnow

Given the manner of his success at Norwich he wasn't going to be an overnight success. He's trying to build a team of youngsters. Lerner shouldn't have been surprised that he's not taken Villa to the brink of the champions league.

I think he's just too fucking tight or stupid. Although I'm glad we've signed players, this window has shown our management up to be extremely short sighted and reactionary. Bear in mind these players have all got to learn how to play together and it may take some time for Pardew to work out exactly how best to play them, so it may actually be some time (? too late) to see any real benefits.

Recovery nurse I used to work with was suspended while a police investigation into her sending threatening texts to a colleague was underway. She still is actually and will almost certainly be sacked now after being found guilty.
