Well, before I do anything else, lets address this. At loggerheads means "engaged in a disagreement or dispute", so I'm not sure where your confusion lies? It will become Medicaid/Medicare. The services provided free at the point of delivery will be of such a standard (thanks to underfunding, understaffing etc.) that expensive private healthcare with be the attractive option.
